The form must be completed in full. In particular the reference (ie paper) must be correctly listed on the form using the format given in the referencing section. It is important to include an email address so we can make contact with you in case of difficulty.

Forms may be faxed or posted to Civil-Comp Press (contact details). Permissions will only be sent out by post cannot be faxed back.
